You have previously received a permanent student Identification card that will be used instead of the paper nametags at each retreat. 

If you lost your ID card, you may order a replacement for a small fee.

For the ID card, we require your personal photo. Please upload your photo where indicated below, and place your order for $25 – then your image will be saved to create the ID card. The photo will be on the card along with your customer number from the MasterSha store. 

Please provide a clear photo, with white background. The photo should be of your face only (similar to passport photo)
